This section deals with oral drugs that offer symptomatic therapy which allows the patient to obtain valid erections. The most effective drugs in this section are selective phosphodiesterase type 5 inhibitors, which are universally considered to be the first-line treatment of ED. The action of all these drugs consists in improving the inflow and trapping of blood within the cavernous bodies (the erectile structures of the penis), and determining a better quality of erection. These drugs “enhance” the physiological dynamics of erection and therefore act only in the presence of an adequate sexual stimulus: this translates into an excellent acceptance by the patient and the partner. The molecules currently on the market are sildenafil, vardenafil, tadalafil and avanafil, which have different speeds of action and duration depending on the needs of the patient. Of note is the new arrival of sildenafil based orodispersible film.
